Early Childhood Health Program Specialist
(Health Program Specialist I)
Would you like to contribute to ensuring families and children have access to the resources they need to reach developmental milestones? The Division of Family Health is looking for a driven, detail-oriented individual to fill their role as an Early Childhood Health Program Specialist.
Job ResponsibilitiesProvide community outreach at events, clinics, and with collaborating partners to meet new families and collect contact information and potential referral needs
Counsel clients and prospective clients; screen referrals, provide community outreach
Communicate on the phone daily taking referrals and assigning to a Care Coordinator in a professional and timely manner
Provide clients with information concerning rights, options, benefits, services, goals and expectations
Refer client/patient to internal care coordination services to meet needs of clients
Decide upon the need for
Enter data into a computer system, retrieve, and/or correct previously entered data
Conducts or participates in day-to-day development and monitoring of contracts, subrecipient agreements, and grants, in support of the larger program functions
Engage in finding new partners
Provide
Ensures compliance with applicable federal and/or state laws, regulations, and/or agency rules, standards and guidelines, etc.
Other duties as assigned
Minimum QualificationsExperience in customer service
Experience working in an office environment
Experience performing outreach services and taking referrals, preferably in relation to health services
Ability to use computer systems to perform data entry
Valid Driver License
Preferred QualificationsExperience working in the Early Childhood field, where children are screened for developmental milestones
Ability to speak, write, and read fluently in Spanish

Schedule
Monday – Wednesday from 8:00AM to 5:00PM
Physical RequirementsTypically, the employee may sit comfortably to perform the work; however, there may be some walking; standing; bending; carrying light items; driving an automobile, etc. Special physical demands are not required to perform the work.
Working ConditionsRisks found in the typical office setting, which is adequately lighted, heated and ventilated, e.g., safe use of office equipment, avoiding trips and falls, observing fire regulations, etc.
