The Bureau of Clinical Services, Mental Health Services is seeking to hire a Program Specialist I. The individual hired for this position will coordinate discharge planning for clients with mental health needs and assist clients in transitioning, to include: coordination of continued mental health services in the community, organizing transportation and follow-up appointments as needed. Duties also include teaching psycho-educational classes.

The Ideal Candidate:
The ideal candidate will have:Experience in coordinating discharge planning for clients with mental health needs
Skill in working with patients, team members, community partners, and family members
Knowledge of Medicaid and SSI applications
Knowledge of community resources and services
Knowledge of PASRR Preadmission Screening and Resident Review
Skill in teaching Psycho-educational classes
Knowledge of how to access community housing resources
Strong organizational and interpersonal skills
Ability to be a team player and work effectively with staff and the community
Knowledge, training and experience in behavioral science, social services and/or social work
Skill in handling stressful situations with tact and professionalism.

Preference:Education and experience in behavioral science, social services and/or social work.

Coordinate discharge planning
Coordinate, review, and prepare Medicaid, SSI and nursing home evaluation forms
Teach Psycho-educational classes
Identify clients needing discharge services and gathering necessary information for benefits and placements
Coordinate with primary therapist and outside agencies regarding discharge plans
Provide consultation, make recommendations, give appropriate advice, and/or facilitate decisions.
