Medical Homes Program Specialist
Health Program Specialist III
Do you have a knack for collecting data, formulating strategic plans, and coordinating with stakeholders to implement initiatives? Do you want to partner with stakeholders at multiple levels to promote access to medical homes for families with children in Utah? The Division of Family Health needs you to join us as a Medical Homes Program Specialist.
Job ResponsibilitiesReview survey information and current data regarding populations that have a medical home vs. those who do not have a medical home
Identify data needs and create surveys to gather necessary data regarding current practices around medical homes in various settings
Identify partners and collaborate to further work currently being done in Utah to increase clients who identify themselves as having a medical home
Work with primary care practice teams on interpersonal and relational dynamics that improve engagement with families
Work with providers to implement quality improvement initiatives and collect data using available tools
Participate in learning opportunities sponsored by HRSA related to medical home
Identify and implement evidence-based strategies and set goals for improvement
Provide reporting for the annual Title V MCH Block Grant report/application
Provide consultation, make recommendations, give appropriate advice, and facilitate decisions
Effectively identify and communicate critical issues to the Early Childhood Utah program manager and team
Support the Early Childhood Utah Program
Minimum QualificationsAt least 2 years of experience in Public Health or Early Childhood Education
At least a year of experience collecting and analyzing Public Health data
At least a year of experience implementing quality improvement initiatives in Public Health
Experience working across multiple agencies/organizations, systems, and with various stakeholders including administrators, private providers, managers, and direct service providers
Ability to use automated software applications
Experience or ability to be proficient in Microsoft Word, Excel, and PowerPoint, and Google Suite
Excellent collaborative and interpersonal skills with ability to exhibit flexibility, use
diplomacy, and be a team player
Preferred QualificationsBachelor’s degree in Public Health, Business Administration, or Early Childhood Education or another applicable field OR 4 years of experience in Public Health or Early Childhood Education
Prior experience working with Utah Medicaid policy and eligibility
Experience working in Utah’s Early Childhood mixed delivery system
ScheduleFull-time; 40 hours per week; Monday – Friday with remote days being Monday and Tuesday, and in-office days being Wednesday – Friday. Some flexibility in schedule may be required to meet agency needs.

Supplemental InformationEffective July 1, 2021, in-state work rules require all work to be performed while physically within the state’s borders.
Continued employment is contingent upon the successful completion of a fingerprinting background check.
Risks found in the typical office setting, which is adequately lighted, heated and ventilated, e.g., safe use of office equipment, avoiding trips and falls, observing fire regulations, etc.
Typically, the employee may sit comfortably to perform the work; however, there may be some walking; standing; bending; carrying light items; driving an automobile, etc. Special physical demands are not required to perform the work.
