Case Management Specialist
Do you have case management experience? The Utah Department of Health and Human Services needs you as a Case Management Specialist to provide case management services to eligible Ryan White Part B clients. You will work with the most vulnerable to be lost to care or at risk of being non-adherent to their medications and those residing in rural locations. You will be helping those living outside the Wasatch Front to provide access to services that have presented barriers in the past. You will provide help in an outreach setting such as in private-public meeting spaces, in client’s homes, over the phone or using other technology. You will respond to technical assistance requests and will provide on-site technical assistance to Ryan White Part B internal and contracted case managers. Under the direction of the Case Management Coordinator, the Case Management Specialist will conduct contract-monitoring activities on a regular basis. These activities include, case management case note reviews and session shadowing. You will also be responsible for all Ryan White Part B client/community engagement activities.

Responsibilities
Principal Duties
As the Case Management Specialist you will:
Provide case management services to people living with HIV who are eligible for Ryan White Part B services.
Provide access to services to those living outside the Wasatch Front.
Respond to technical assistance requests and provide on-site technical assistance to Ryan White Part B contracted case managers.
Coordinate Ryan White Part B client/community engagement activities.
Minimum Qualifications
Minimum Qualifications
Has 3+ years HIV case management experience.
Has a Bachelor’s degree in social work or behavioral science with social work emphasis.
Has prior experience facilitating group-level interventions.
Has prior experience with public speaking.
Has working knowledge of the Ryan White Part B Program.
Preferred SkillsExperience working with people living with HIV
Case Management experience
Experience working with community partners and clients
Strong writing skills
Public Speaking
Preference may be given to applicants with any of the above.
Supplemental Information
Working Conditions:
The typical office setting for this job is adequately lighted, heated and ventilated. However, you may encounter some risks that are associated with any office setting, for example, the safe use of office equipment, avoiding trips and falls, observing fire regulations, etc.
Standard Schedule: Monday – Friday, 8:00 am to 5:00 pm
Travel is required
Physical Requirements:Typically, the employee may sit comfortably to perform the work; however, there may be some walking; standing; bending; carrying light items; driving an automobile, etc. Special physical demands are not required to perform the work.
This position is currently a hybrid of both in-office and remote work days. Please note, a position’s eligibility for remote work is established by agency management and is subject to change at their discretion at any time and for any reason.
