Principal Duties
A Case Manager at the Utah State Hospital will

support patients in following individualized treatment plans and identifying and achieving case management goals.
help coordinate care with other health care providers.
assist patients in accessing community resources related to discharge and successful transition to the community.
ensure patient participation in accessing community resources as much as is feasible to help the patient learn about the resources and skills to access resources.
assist treatment teams and community liaisons to assess, plan for, and provide needed mental health, health, substance use and related services for patients as needed.
facilitate effective collaboration by educating community partners and resource agencies about the Utah State Hospital, process, policy and procedures.
document patient services with hospital electronic records within Utah State Hospital standards.

The Ideal Candidate
Preference will be shown for a candidates who:are licensed Social Service Workers or Certified Case Managers.
have previous experience coordinating client services.
are willing and able to become a Certified Case Manager within 6 months of hire.
A candidate who is minimally qualified for this position:has a high school diploma or equivalent.
has education, training or experience in human services or a related field.
has experience working with children and youth in an inpatient mental health environment.
is willing and able to become a Certified Case Manager within 6 months of hire.

Risks found in the typical office setting, which is adequately lighted, heated and ventilated, e.g., safe use of office equipment, avoiding trips and falls, observing fire regulations, etc.
Typically, the employee may sit comfortably to perform the work; however, there may be some walking; standing; bending; carrying light items; driving an automobile, etc. Special physical demands are not required to perform the work.
